How I approach knee pain

I see a range of knee problems, including patellofemoral pain, arthritis, meniscal injuries, dislocations and acute sporting injuries such as ACL injuries.

The approach depends on what we find. An acute injury needs a different approach to pain that has been present for a long time, but in both situations I want to understand the whole picture rather than simply treating the knee in isolation.

I'll look at things such as strength, movement, activity, loading capacity and what you want to get back to doing.

Common presentations

These are the presentations that come through the door most often, and each one behaves differently under load.

Patellofemoral pain

Pain around or behind the kneecap, often worse on stairs, hills and after sitting.

Arthritis

Manageable with the right loading. Strength and capacity matter far more than what a scan shows.

Meniscal injuries

Frequently manageable without surgery, depending on how the knee responds to graded loading.

ACL and acute sporting injuries

Accurate assessment at the start shapes the whole rehabilitation timeline and return to sport.

People often worry that using their knee is causing further damage. Understanding its actual capacity changes that.

Strength, movement and loading built back up in stages, which is what lets the knee handle a normal week again.

Common questions

How long is an appointment and how many will I need?
The first appointment is long enough for me to take a full history, examine you and explain what I have found before you leave, rather than a rushed slot. Most tendon and patellofemoral problems I see improve within four to eight weeks. I will give you an honest estimate at the assessment rather than an open ended plan, and if you end up needing fewer sessions than I thought, I will say so.
Do you treat meniscus tears and post surgical knees?
Yes. Runner's knee treatment, meniscus tear treatment, knee replacement rehabilitation and ACL rehabilitation are all handled here, with the plan matched to the tissue and the stage.
Why do you assess my hip and ankle for knee pain?
Because the knee sits between them and takes the consequences of how both behave. A stiff ankle or a weak hip changes what the knee has to absorb every step. Treating only the painful joint is why a lot of knee pain keeps returning.
Can I still run with knee pain?
Often yes, with modification. Complete rest tends to reduce capacity further. The assessment establishes what dose your knee currently tolerates.
